Incentive to participate: You and/or your covered spouse can each receive $50 when you complete your Well-Being Assessment (WBA). Just make sure you complete it by December 31, 2011. A check will be sent to the participant from the Pension, Hospitalization and Benefit Plan of the Electrical Industry.
